Heritage Park is a long term care community located at 2001 Hobson Rd. The Heritage Park Campus includes independent-living garden homes, assisted-living apartments, Moving Forward Rehabilitation and long term nursing home care.
Being located in close proximity to area hospitals, physician offices, restaurants and shopping makes Heritage Park a great place for seniors to call home. The campus offers a continuum of care that allows residents to stay in a familiar community as their medical needs change. Heritage Park can meet the needs of their residents on a short term or long term basis.
Approximately 10 years ago, after making multi-million improvements to the long term care center, American Senior Communities began construction of independent and assisted living housing. The anticipated goal was to provide seniors with a housing choice that would meet their healthcare demands as their medical needs changed. Providing senior living choices has proven to be a necessity in the Ft Wayne/New Haven area.
The garden homes on the campus are positioned to border the assisted living apartments on 3-sides. These 48 homes range in size from1bedroom/ 1 bathroom to 2 bedrooms/2 bathrooms. The property is maintained by the Heritage Park Maintenance Team under the direction of Kimball Tetzloff. These men not only oversee snow removal and lawn care; but also perform in-home repairs. This alleviates the frustration of calling an outside service, waiting for them to make a service call- then allowing strangers to enter the home to provide the needed repair services. Using Heritage Park staff ensures security within the community and is just one of many features the campus is proud of!
The 32 assisted living apartments on campus are all located in one building located directly behind the long-term care center. These private apartments offer a studio-style layout as well as a 1-bedroom/1-bathroom floor plan. Seniors residing in the apartments have the security of knowing they have staff available on a 24 hour basis. Individual health plans are developed to meet the varying needs of those living in the apartments. From medication reminders to hands on nursing care; the assisted living team can meet the needs of those seniors requiring assistance.
Both the Garden Home and Assisted Living residents have the opportunity to dine in the AL dining room. 3-nutritious meals a day are provided. Those living in Garden Homes that prefer to not come to the dining room but would like a meal provided are able to have meals delivered to their door by the Dietary Staff.
The Garden Homes and Assisted Living Apartments are under the direction of Greg Hevel who has performed duties as the General Manager at Heritage Park for almost 10 years. Originally from New Haven; he currently only lives about ½ mile from the facility. This allows him the opportunity to be flexible with availability to meet with residents, families and outside inquiries interested in learning more about what Heritage Park has to offer. Greg states that it is “really about dealing with people; making sure everyone is happy and taking care of issues/concerns.”
Being the largest in the area; the long-term care skilled nursing building is currently licensed to provide nursing service to 180 beds. Boasting 9 halls and 5 dining rooms- more than 230 employees ensure comprehensive individualized care is provided to the residents. Two nurse practitioners work in conjunction with Dr. Sidel and Dr. VanDenDriessche to provide assessments and medicinal/medical interventions timely. The NPs are in the facility on a daily basis to see residents and are available to meet with residents and families to answer questions.
Heritage Park employs 17 therapists that are able to provide skilled speech, occupational and physical therapy. Having facility based employees allows for continuity of care. The residents are able to get to know their therapists and the therapist is able to individualize modalities provided. The therapy department is able to not only provide skilled services to residents at Heritage Park- they are also able to perform therapy on an out-patient basis. More than 80% of the population working with the skilled therapy department returns to a fully functional lifestyle. Yet another reason Heritage Park stands apart from other facilities in the area.
Heritage Park has been a member of the New Haven Chamber of Commerce since 2009. Mr. Hevel has attended meetings and reads the Chamber Newsletter to stay informed of what is happening in the New Haven area. He states that “the city of New Haven is growing and we realized there are a lot of areas outside of Ft Wayne that would benefit from the services our campus has to offer. New Haven is one of those areas.”
Kris Dowell, Marketing Director for Heritage Park, spends her time educating the public about the benefits available at the Heritage Park Campus. She meets with physicians, discharge planners, attorneys, senior groups and other potential referral sources in an effort to provide information helpful to have when the time comes to make alternate living arrangements as we grow older.
